For anything that touches money, email, customers, or files you can't get back, in-the-loop is the answer. Watching a fast machine and hoping to catch it mid-mistake is not supervision. It's spectating.
Here's the tension. Put a human approval on EVERY step and you've built the thing everyone already hates. The word people on Reddit keep reaching for is babysitting. One user described spending the day next to his agent pressing approve like a cartoon bird tapping a key. That's not human in the loop. That's human AS the loop.
The fix is where the gates go. Not on every step. On the consequential ones.

It has standing orders about what it may never do alone: send, spend, publish, delete, grant access. When it reaches one of those doors, it writes up what it wants to do and why, and it stops.
